How To Register As A New Patient

All new patients wishing to register with the Practice are asked to download and completing the 2 forms below

Please Note: Once you have completed the forms online you will become a registered patient at Highview Medical Centre. However, to fully activate your registration with us you will need to come in to the surgery with proof of address. Once this has been completed you will be able to access our full range of services including the ability to book appointments & request medication on-line.

Once I Have Registered As A Patient

Once we have accepted your registration as a new patient you will be invited to make an appointment for a new patient medical with the Practice Nurse or Healthcare Assistant. This is important so that we are able to record full details of your medical history, take up to date blood pressure readings as well as discussing your general health with you. You will be asked for a urine sample for testing as well as information about family history of illness and chronic conditions.

Children under 5 years of age do not need to have a medical but parents will be asked for details of all immunisation dates. We will ask that you bring in the red book for each of the children so that a full immunisation history can be taken. The doctors at Highview encourage all children to have full immunisations and appointments can be made in advance.

Temporary Registration

You can register temporarily with a GP near where you are staying if you are  ill or need medical advice while you are away from home, for example, working, studying or on holiday. You still remain registered with your permanent GP but you can see your temporary GP, please speak to reception if you need to be seen as a temporary patient
